Parents of children with Asthma, Anaphylaxis, Diabetes, Epilepsy or other medical conditions of concern, please complete the appropriate form (below) and return it to the school office. It is also very helpful to notify the office if there have been any changes in your child’s health status i.e., the condition has changed, from previous years. New forms are required at the beginning of every school year.
